What is an Anticoagulation Clinic?
The Anticoagulation Clinic, also known as Coumadin Clinic, is a simple, convenient way to monitor and assist patients taking warfarin. Our clinic is staffed by trained, clinical pharmacists to provide education and dosing adjustments of warfarin under the supervision of the referring provider.

Why an Anticoagulation Clinic?
Patients taking anticoagulants need to be aware of the potential for serious side effects that can occur if the medication is not properly monitored. Some of these side effects can be life-threatening. Research has shown that participating in an anticoagulation clinic may decrease serious complications from anticoagulation medications.

What are the benefits of participating?
● Decreased risk of potentially life-threatening complications
● Immediate PT/INR results (Prothrombin Time/International Normalized Ratio)
● One-on-one contact with a clinical pharmacist
● Better anticoagulant monitoring and management
● Increased access to education and support

How does it work?
Patients of the clinic are seen by a member of our staff for appointments. During your appointment, a small drop of blood will be collected from a finger prick to run the test. Your PT/INR results are available in less than one minute. The results are reviewed with you, your medication will be adjusted as needed, and easy-to-read, written instructions are provided. The results are also shared with your doctor. At each visit, education is also provided on complications, diet and drug interactions related to taking warfarin.

What is Hospice?

Hospice exists to provide an alternative to acute care for the terminally ill and their families. The focus is to provide support and care for persons in the last phases of an incurable disease so that they may live as fully and comfortably as possible. Hospice affirms life; however, acknowledges death as a normal event.

Do I qualify for Hospice care?

Hospice cares for all ages and all diseases. Our individualized care is beneficial for anyone with any end-stage illness or a combination of chronic conditions, including heart, lung or kidney disease; cancer; dementia/Alzheimer’s disease; and neuromuscular disorders, such as Multiple Sclerosis, ALS and Parkinson’s disease.

Will Hospice cure my illness?

Hospice offers palliative or comfort care rather than curative treatment. We focus on pain and symptom control that enable the patient to live as fully and comfortably as possible.

What does Hospice do?

Hospice treats the physical, emotional and spiritual needs of those they serve, supporting patients, families and caregivers with education, training and counseling. Services are provided using a team approach by professionals including physicians, pharmacists, nurses, social workers, nutritionists, bereavement counselors, chaplains and volunteers.

How much does Hospice cost?

The costs of care, medicines and equipment needed to control the symptoms of your terminal illness are covered by Medicare, Medicaid and some insurance plans. We welcome all patients in need of Hospice care and provide all possible billing assistance to you. Our staff can help in identifying resources and programs available.

What is covered? Where can I get care?

Hospice provides supplies, medications and equipment needed to maintain quality of life in your home – wherever you call home, including assisted living residences and nursing homes. We are contracted with Fairhaven Community, The Inn at Westbrook and Wyandot County Skilled Nursing and Rehabilitation Center.

Hospice professionals also collaborate with the WMH inpatient staff during hospitalizations to provide the best care possible. The hospital staff ensures our patients’ primary care needs are met, while daily Hospice visits ease symptoms and provide important support. An inpatient Hospice stay is typically brief: days, not weeks, to establish pain and symptom management.

How long can I be on Hospice?

There is not a time limit for Hospice care as long as the patient meets eligibility requirements. Patients and families have the ability to revoke Hospice at any time if a condition or situation changes.

What about my family?

Hospice continues to care for the family even after the death of the patient, providing one-to-one grief counseling, support groups, social opportunities and community workshops.

Can I call Hospice at anytime?

Hospice is always here for you. Help is just a call away, 24 hours a day, 7 days a week.

Choosing Hospice care can be a difficult decision for patients and their families. Often people hesitate to explore the hospice option because they are confused about Hospice services or they believe it is only for the last days of a patient’s life. SAFE, EFFICIENT MEDICAL TESTING

The Wyandot Memorial Hospital laboratory performs clinical tests for diagnostic and treatment purposes. Our state-of-the-art lab is staffed with highly trained medical technologists. We perform blood tests, urine testing, drug testing, STD testing, virus identification, and body fluid analysis, in addition to other services.

For your convenience, we perform lab testing with your healthcare provider’s order – whether they’re on the local medical staff or at another facility. So even if you do not see a doctor here, we can still do your lab work.

Most lab services are performed on an outpatient basis. We strive to analyze results as quickly as possible without sacrificing accuracy. TREATMENT FOR CANCERS & BLOOD DISORDERS
The hematology and infusion center at Wyandot Memorial Hospital provides treatment for patients with various cancers and blood disorders, as well as other conditions that can be treated through infusion. The center is run by a team of skilled nurses and medical professionals who strive to create a warm, comfortable environment where approximately 9,000 infusions are performed annually.

If this is your first time visiting an infusion center, our staff will guide through the process. Receiving infusion is a generally simple process. In most cases, all you have to do is sit in a comfortable chair while the medication travels into your veins through an IV. Duration of treatment varies based on the purpose for the infusion.

What is infusion Therapy?
Infusion therapy is the administration of medication directly into the bloodstream. It is more effective and faster acting than oral medication, which is why it is generally only used to treat more complex conditions. Chemotherapy is a treatment administered through infusion therapy. Infusion therapy is also commonly used to treat patients suffering from dehydrtion, ulcerative colitis, Crohn’s disease and anemia.

Wyandot Memorial Hospital’s Patient Navigator provides one-on-one guidance and assistance to patients, families and caregivers and helps with many needs. The principal function of the Patient Navigator is to eliminate any barriers to timely screening, diagnosis, treatment and supportive care for each patient while also promoting smooth and timely continuity of care. The Patient Navigator works with a variety of patients – not just oncology patients or patients with chronic diseases – but rather any patient facing diagnostic testing or requiring assistance in any aspect of their medical care.

How can the Patient Navigator help me?

  • Arrange transportation to and from appointments
  • Identify resources for financial assistance including medication, co-pay assistance and other community resources
  • Coordinate continuity of care for WMH patients who need to have surgery or treatment at another facility (i.e. The James Cancer Hospital, etc.)
  • Assist with keeping track of doctor visits, tests and other appointments
  • Help completing medical forms including those relating to insurance, Medicaid and Medicare
  • Answer general questions about health insurance
  • WMH Patient Navigator is a Certified Application Counselor for the Health Insurance Marketplace and can enroll patients and help them purchase health coverage through this program.
  • Provide information on and assistance with Advance Directives (Living Wills and Healthcare Power of Attorney forms)

WE’RE COMMITTED TO IMPROVING QUALITY OF LIFE

The physical therapy and occupational therapy programs at Wyandot Memorial Hospital are run by a team of professionals who build customized programs to fit the needs of each and every patient. We have three different programs for both therapies – inpatient, outpatient, and at-home services. Through strategic planning and dedicated effort, we help our patients regain their independence and enjoy a higher quality of life. We also provide one-on-one care for our patients, and this approach enables us to offer the quality care that every patient at our facility deserves.

Both physical therapy and occupational therapy have an incorporated Swing Bed Program (SWP). SWP is for patients who no longer need intensive acute therapy in the hospital but are not quite ready for home. This is an in-between stage between inpatient and outpatient that provides an opportunity for patients to continue recovering and training for a safe return home.

Physical Therapy

The goal of physical therapy is to help patients regain independence and mobility after an injury or illness renders them incapacitated. Physical therapy is more general than occupational therapy; in this program, we want to help patients improve all aspects of their lives, not just one particular area or skill. Physical therapists have many avenues in which they can achieve this.

We have a team of licensed physical therapists and licensed physical therapy assistants. Our team is experienced in working with all patients from pediatric to geriatric. Every patient works closely with a physical therapist and physical therapist assistants in order to build a recovery program tailored to their unique needs.

Occupational Therapy

In outpatient therapy, therapists learn more about each patient’s goals for recovery and build a more focused therapy program. For example, if a patient was a musician who suffered a hand injury, the therapy would have an emphasis on rebuilding hand strength and finger dexterity.

Our team includes three occupational therapists and three occupational therapy assistants. Like physical therapy, occupational therapy has inpatient, outpatient, home services programs and works with patients of all ages.

HELPING PATIENTS OF ALL AGES BREATHE EASIER

Respiratory therapy focuses on treating conditions of the cardiopulmonary condition. More specifically, it is most often used to help patients manage symptoms that impede their ability to breathe. At Wyandot Memorial Hospital, our respiratory therapists have extensive knowledge of the pulmonary and cardiovascular systems. A respiratory therapist also provides around-the-clock care for emergencies.

What to Expect During Respiratory Therapy

Like most therapy programs, respiratory therapy is customized based on the unique needs of the patient. Therapy can be done in an inpatient or outpatient setting. Most patients who are referred to respiratory therapy are those with chronic lung conditions such as cystic fibrosis and emphysema.

Services provided by respiratory therapists include:

  • Assessing lung strength and capacity
  • Helping patients prepare for travel
  • Exercise training
  • Managing symptoms of chronic diseases

Respiratory therapy goes on for as long as the patient needs it. Patients with serious conditions may require respiratory therapy for the rest of their lives, but the services in therapy should help them improve their quality of life.

When Is Respiratory Therapy Necessary?

As stated above, respiratory therapy is provided in emergency situations and for patients who receive a physician’s referral. Children, adults, and seniors can benefit from respiratory if they have a chronic lung condition.

The following are some of the disorders treated by respiratory therapists:

A good night’s sleep is an essential part of healthy living, but for patients diagnosed with a sleep disorder, rest can be hard to come by. There are various types of sleep disorders, but the most common is obstructive sleep apnea “OSA”. This disorder impacts 1 in 4 adults in the United States.

What is Sleep Apnea?
Sleep apnea is a pause in breathing for a short period of time during sleep. This can happen hundreds of times during the night. These disruptions can cause your rest to be less refreshing and your body is unable to reach deep restorative sleep, leaving you tired during the day.

SPEECH LANGUAGE PATHOLOGISTS IN OHIO

Difficulty speaking is a frustrating problem for people of any age. Speech disorders are common in children and can be a major roadblock in their mental and social development. The speech language pathologists at Wyandot Memorial Hospital help patients of all ages overcome speech and swallowing disorders.

Speech language pathologists help patients with a variety of problems. In addition to speech disorders, they can also help people with other communication difficulties like dyslexia and auditory processing disorder. The goal of speech therapy is to help patients fully engage and interact with the world.

We often treat patients referred to us for:
• Expressive and receptive language disorders
• Difficulty with language fluency
• Difficulty with articulation
• Auditory processing disorders
• Oral-motor dysfunction
• Augmented communication
• Social disorders

When to Look Into Speech Therapy
It can be difficult for a parent to determine when their child needs speech therapy. Every child develops differently, and it’s not always easy to figure out when they might need help. There are some markers for speech development in children that can point you in the right direction.

Your child may need speech therapy if:
• People have a difficult time understanding what they’re saying
• Your child takes a long time to say things
• Your child is silent for a long time before responding to simple questions
• Your child is over two and is having a hard time putting two words together
• Your child has difficulty making certain sounds like “th” or “ch”
• Your child has a stutter
• Your child has a hoarse voice

There are certain milestones for children as they age which should give you an idea of how their speech is developing. Talk to your doctor if you are concerned that your child’s communication skills are not where they should be.

VOLUNTEER CHAPLAINS

Chaplains are spiritual caregivers with advanced training in supporting people during times of illness or emotional distress. Wyandot Memorial Hospital understands the vital role that faith and spirit have in the lives of our patients and their families.

Volunteer chaplains are available to any patient or family member, Monday through Friday (typically in the mornings). Chaplains are also on-call evenings and weekends for urgent situations and emergencies. All of our volunteer Chaplains wear badges with their picture and name and they adhere to strict hospital confidentiality policies.

When to Request a Visit from the Volunteer Chaplain

  • When you need emotional support
  • When you desire prayer or spiritual guidance
  • When you are facing hard decisions or ethical issues
  • When you desire a specific rite or ritual and your spiritual caregiver is not available (or you don’t have one)
  • When there is a health or family crisis
  • When you are wrestling with spiritual issues such as grief, inability to pray, sense of being abandoned, or when you experience lack of hope or question the meaning of life
  • When you need information about community religious groups and/or resources

How to Request a Visit

Ask your nurse or any member of our medical team to contact the volunteer chaplain for you; our volunteer chaplains typically round in the mid-morning hours but special requests can sometimes be accommodated.

Our SportsCare for Injury Care and Performance Improvement

SportsCare at Wyandot Memorial Hospital partners with local schools for athlete-centered injury care and performance improvement training techniques.

SportsCare provides a certified athletic trainer to Carey High School, Upper Sandusky High School, and Mohawk High School as part of the athletic staff. As a healthcare professional, an athletic trainer is responsible for injury and illness prevention, wellness promotion and education, emergent care, examination and clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions.

SportsCare athletic trainers work with coaches and athletes in all sports for on-site medical care for practices and games or matches. They treat and manage sports injuries, illnesses and conditions; and coordinates care with UrgentCare, the Emergency Department and medical and rehabilitative specialists at Wyandot Memorial Hospital as needed. They also provide concussion prevention and management, and streamline return-to-play to keep athletes safe and in the game.

YOUR STEP BETWEEN HOSPITAL & HOME

Patients with serious injuries and illnesses are not always ready to return home right away even after their intensive acute care has ended. The Swing Bed Program (SWP) provides patients with an opportunity to prepare for life at home through skilled nursing and therapy services. Although the SWP is different from the hospital services patients received before, they still remain in the same room and work with the same members of our medical staff.

Helping Patients Achieve Their Goals

Physical and occupational therapy are the main components of the SWP. Patients work with a skilled physical or occupational therapist who creates a personalized recovery program based on the patient’s individual needs. Staff members help patients create short-term goals that make it easier for them to recognize the progress they make during recovery.

Some goals for the SWP include:

  • Increasing muscle strength
  • Learning pain management techniques
  • Learning how to use medical devices such as crutches or wheelchairs
  • Learning optimal breathing techniques
  • Improving range-of-motion
  • Regaining proficiency in small tasks such as writing, using phones, etc.
  • Creating a new diet
  • Learning ways to safely move around in the home environment while physically impaired
  • Learning how to use adaptive equipment for increased ease with daily tasks, such as dressing

The patient’s care team meets weekly to discuss the patient’s progress and estimated recovery time. Patients and family members are encouraged to attend these meetings and ask questions.

Life in the SWP

Once in the SWP, patients are encouraged to start taking a more active role in their lives and participating in physical and occupational therapies. Hospital staff is always nearby and available to help with any tasks a patient may be struggling with. We also have many activities and events so patients can entertain themselves while staying in the SWP. As patients start regaining their independence through therapy, there will be more opportunities for them to move around and get used to a more active routine. Visits from friends and family are encouraged.

Medicare covers services in the SWP, meaning the program follows all Medicare-designated guidelines. All patients are given a 48-hour notice before discharge from the SWP. If the patient is returning to their home instead of a nursing facility, we can provide a home safety check before discharge if the patient’s therapist thinks it will be necessary. During this check, the patient, a family member, and a therapist examine the patient’s home to ensure a safe discharge. The therapist may have recommendations to make the home environment safer and more accessible for the patient.

UrgentCare is a walk-in clinic where no appointments are necessary. We provide same-day diagnosis and treatment for a variety of non-life-threatening injuries and illnesses. Most insurances are accepted, so be sure to bring your insurance card with you. Co-pays may be necessary.

What Does UrgentCare Treat?

UrgentCare is not emergency care. If you are experiencing a serious medical event, call 911 immediately. The purpose of UrgentCare is to help people with important, but non-life-threatening medical concerns as soon as possible.

WE’RE HERE TO HELP YOU STAY FIT & HEALTHY

Wyandot Memorial offers a multitude of health and wellness services through the Population Health and Wellness Department in our Wellness Center.

Some services include Cardiac and Pulmonary Rehabilitation, our diabetic self-management services, Living With Diabetes program, nutrition services, various support groups, the Matter of Balance program, and wellness gym membership services and SilverSneaker partnership.

Cardiac and Pulmonary Rehabilitation

Cardiac or Pulmonary Rehabilitation is a physician-ordered, medically-supervised program of exercise, emotional and social support with lifestyle modifications to help the heart and lungs regain strength. Rehabilitation reduces the risks of cardiovascular disease, improves pulmonary function and decreases mortality while providing a safe environment.

Our cardiac and pulmonary rehabilitation team include our Medical Director, a Registered Nurse, Registered Dietitian and an Exercise Physiologist. All staff members are certified in Advanced Cardiac Life Support and work together to determine blood pressure, interpret EKG monitoring and to further advance each patient. Continuous EKG monitoring is available based on your ordered diagnosis and insurance coverage. This program lasts approximately 12 weeks as advised by the ordering physician.

Diabetic Education

The mission of the Diabetic Team at this facility is to partner with the diabetic patient, physician and patient’s family to provide quality and comprehensive diabetes self-management education and tools. This will include empowering the patient to better manage his or her disease, improve their quality of life and prevent complications of diabetes.

The goals of the Diabetic Team are to provide knowledge and skill training, help individuals identify barriers to treatment, teach problem solving and coping skills to achieve self-management, and behavior change as needed. Diabetic Team members include the patient and family, primary care practitioner, diabetic teaching nurse, pharmacist, dietician, and exercise physiologist.

The diabetic teaching nurse works with each unique individual to identify his or her needs related to diabetes self-management. Education needs, behavioral goals and follow-up plans are established after a review of the physician’s health assessment, completion of a nurse assessment and review of a complete foot exam.

  • Living with Diabetes offers diabetic self-management education training and support. Living with Diabetes is a 6-hour physician-ordered, multidisciplinary educational course with yearlong follow-up by the diabetic teaching nurse. After the full year, Medicare patients are eligible for two hours of continued nursing education per year if order by the physician.
  • Individual education is offered for patients with special needs such as vision problems, hearing deficits, physical limitations, cognitive impairment, and language limitations.
  • The Diabetic support group has roundtable discussions and guest speakers throughout the year.
  • Other services offered include continuous glucose monitoring, glucometer instruction and insulin instruction.

Nutrition Services for Creating a Healthy Diet

Our Registered Dietitian can help you generate a healthy meal pattern to help you meet your personal goals.

  • Outpatient Nutrition Services focuses on understanding your needs, routines, how to change unhealthy habits and gain healthy behaviors.
  • Nutrition counseling and Medical Nutrition Therapy (MNT) diet education are offered for patients and families. This may include but are not limited to diabetes education, weight management, cardiovascular disease, renal disease, malnutrition, weight gain, cancer and chemotherapy, sports nutrition and/or maintaining a healthy diet.
  • Portion Control and Holiday Recipes

Wellness Center Gym

The Wellness Center offers exercise programs for both healthy individuals and those with medical concerns including pre-diabetes, cancers, hypertension, arthritis or orthopedic concerns and comprehensive weight management.

Wellness Center membership includes an exercise consultation, wellness assessment, use of all equipment, blood pressure screening, full lipid panel bi-annually.

  • General membership fees start at $30/month. Reduced pricing is available for multiple family members and volunteers of WMH.
  • The Wellness Center participates in SilverSneakers and Renew Active to provide use of the gym at no out-of-pocket cost to those whose insurance plans include the benefit. Enrollment through SilverSneakers and Renew Active starts with your insurance carrier. Other Wellness Center services are available to participants for a small fee.
  • Registration is required for Wellness Center memberships and classes.

TREATMENT FOR INFECTED & NON-HEALING WOUNDS

In many cases, wounds will heal on their own over time if properly cleaned and bandaged. However, some serious wounds have a difficult time healing and may even become infected. Additionally, people with diabetes and circulatory problems have a harder time healing wounds than other people. An open wound is nothing to take lightly. If you have a wound that has not shown any signs of healing after two weeks, you will want to have it treated by a professional as soon as possible.

Wyandot Memorial Hospital’s WoundCare provides advanced treatments for non-healing wounds. Our team can help you prevent infections, reduce pain, and explore treatments that will close the wound for good. We are staffed by a Certified Wound Nurse with in-depth knowledge of wound care and the most progressive dressings and wound vacs. We can also help with care, products and resources when caring for a new or established stoma.

STATE-OF-THE-ART DIAGNOSTIC TESTING

Imaging is used to obtain an inside look at the body’s bones, muscle, tissue, and organs. X-rays, which take pictures of the skeletal system, are an imaging service. Wyandot Memorial Hospital offers a variety of high-quality imaging services to assist in diagnosis and treatment. We have invested in state-of-the-art technology to create clear and detailed images so that we can provide patients with accurate information.

A specialist called a radiologist is responsible for analyzing imaging results. Our radiology team is highly experienced and familiar with all of the technology used at the hospital.
